Instructions

1. Prepare the scanning packet in the following order:
   1. Request form
   2. Additional instruction forms (e.g., weighting, unscrambling), if applicable
   3. Answer keys (in order, starting with key “A”)
   4. Student response sheets (face-up with all sheets oriented in the same direction)
2. If you are using half sheets, wrap the request forms around the sheets
3. Use a rubber band or paper clip to secure the packet
4. Drop the packet in the bin at the Scanning counter (for after-hours, please use the drop slot)

Checklist

• This Form.
• Student response sheets.
• If you have multiple keys and want a combined item analysis, or if your questions have different point values, please complete an Item Scramble Form.
• If you have questions with partial credit answers, please complete a Response Weighting Form.
• If you have questions have different point values. Please complete an Item Weighting Form for each form.
• Please check that all sheets that need an Instructor Graded Score have a score and the score is properly bubbled.

Please review your request before you submit it.

As a reminder, Scanning requests are processed within 2-3 business days of receipt.
